The Annual Cycle of Care

In Australia, people with type 2 diabetes are eligible for a GP Management Plan and a Team Care Arrangement through Medicare. These provide access to Medicare rebates for allied health services like dietitians, podiatrists, and diabetes educators.

The Diabetes Annual Cycle of Care is a structured review that covers all the key checks listed above. It’s designed to catch problems early and make sure your management plan is actually working for you.
